The Living Healthy with Diabetes Self-Management Program (DSMP) is for people with diabetes and would like to build confidence in their ability to manage the symptoms of their diabetes and improve their overall health.
This workshop-style program focuses on skills and techniques of self-management through an exploration of the Symptom Cycle, Toolbox of Techniques and weekly Action Plans. Meetings that take place once weekly for 2 ½ hours for 7 weeks and are facilitated by two trained program leaders.
This course will be offered for a total of 7 sessions, which includes a session zero to review online platform basics and confirm interest. Participants are encouraged to attend at least 4 of the sessions in order to experience the benefits.
