Community organizations like Area Agencies on Aging and others are uniquely positioned to address the social care needs of older adults and people with disabilities. Increasingly, healthcare organizations are recognizing the value of these agencies to address community needs and keep people healthy and in the community as long as possible. In central Ohio, the local AAA established a unique partnership with Molina to address the 'social determinants of health' for Molina's patient population. Learn from two of the leaders of this initiative, who will speak to the opportunities, challenges, and lessons learned of implementing a program with a healthcare partner.
